Creating Healthier Communities Together

Together with our partners, we’re working towards a more holistic and equitable system where local services better support all dimensions of well-being for every community member. Social and economic factors like food security, housing and jobs determine more than 60% of well-being. Clinical care addresses symptoms, but the root of well-being begins in the community.

How We Work

Listen to the Community

Every community is unique and hosts a diverse range of well-being satisfaction and health concerns. Each year, PIHC conducts a county-wide survey that tracks well-being trends in the community over time. We use the results to identify the areas of greatest need in our community. To learn more, take a look at My Community Health and Well-Being Monitor.

Create Interconnection

Health happens where and while we live, work and play. Collaborating with communities and creating connections allows us to improve access to resources, build better clinical and community linkages and develop trusted partnerships. The connections we make help our partners to address all the dimensions of an individual’s health. For more details, see Edge of Amazing and PIHC LiveWellLocal.org.

Harness the Power of Partnerships

We believe that everything we need to thrive is readily available within our communities. By fostering connections, sharing common tools and information we’re working to support a collective means for the community to work together to expand the depth, usability and equity of available resources. For details, see Community Outreach and MyLiveWell360.

Addressing the root causes of poor health outcomes creates a happier, healthier community and reduces burden on the medical system.

Factors like access to healthy food, reliable housing and personal safety play a much larger role in well-being than clinical care.

We connect the community with local well-being partners, creating a sustainable approach to our collective health.
